1. Listen to the Voice of Reason
Just before you do something that gets you killed, there is probably a voice in your head that says things like...
"This is probably a bad idea, but..."
"That enemy may not be on his own, but..."
"Driving this far out in the open my hurt, but..."
Learn to listen to that voice. Drop the "...but I'll do it anyway" part, and replace it with, "...so I won't do it".
2. Look for the Smartest Damage Trade
The situation where you hurt the other guy, but don't take damage yourself. That's how the really good players (not me) always seem to be alive and healthy at the end of the battle, yet have still clocked up stacks of damage in the early- and mid-game stages.
If you pull out to hit someone and succeed, AND pull back to hard cover without TAKING damage, you win the major prize. Repeat. Rinse.
If you pull out to hit someone then sit there waiting to reload "for the killing shot", you're at risk. Occasionally, it may be worth the risk, but it's handing over the "smart damage trade" opportunity to the other team, to be used against you. In lower/mid tiers, you may get away with it... sometimes. In high tiers, you get severely punished almost every time.

pizzastorm: This is all great advice. The biggest improvement to your play will
come when you take the time to think about the situation you are in
and make sure you are not doing something you will regret. Map
awareness is one of the biggest things you can have, along with
knowledge of when to "trade damage". In reality, as Officemonkey
points out, you want to get away with doing damage without them
being able to return it. For this, I suggest you look up some
tutorials on our forums in regards to angling your armor and
baiting shots.
