page. On the top right side of the page, underneath the tournament
logo and name, you should see several options (modify team, close
team enrollment, apply for tournament, delete team): 
As the captain of the
team, you will be able to click on the Apply for Tournament. Once
you do that, the team status will change from "Forming" to either
"Accepted" or "Pending Approval," depending on the tournament.
