than one team, it is possible that they may face each other at some
point. The groups are also changed for each round of the
group stage. Again, this is a random process which normally puts
different teams together.

The team does not have the minimum number of players and/or The
team captain did not Apply for Tournament. If the team is not
Pending Approval when registration is closed, the team will be
automatically declined. I would guess the problem was the
second, where the team was not Pending Approval before registration
was closed.
