But basically...
There is two systems, One monitoring each game, on a one game basis. And another monitoring the players recent past.
If either of those hit certain ratios the account will either be banned or flagged blue.
Its very common for accounts to be game banned, with never turning blue. "turning blue is a small window between being ok, and being banned"
Bans escalate for repeat offenders, the bans start at one day and escalate up to permanent.

Reporting friendly fire, Is not needed or accepted, due to "the auto system handles it well"
Disputing friendly fire bans from the friendly fire system, similarly aren't looked into. The system works so bans aren't removed.

And to address the common question of, Why didn't I receive compensation (credits), and get heavy fines (credits) for friendly fire.
Low credit rewards are to make sure the system isn't exploited, If payments were high for friendly fire, players could try driving in front of artillery or sniping tanks for example.
High credit penalties are to encourage players to take aim and not create friendly fire.
