Not the greatest diagram I've ever made but this might explain what happened.
Red indicates the ammo rack location on the O-Ni. Black is the penetration location and blue shows possible shell trajectory. Internal ricochets are possible, which explains why, say, your engine might get damaged from a shell that went into the commander's hatch on your T110E5.
