Grimbru: If a clan manages to capture a Citadel first and then the
surrounding Approaches, each Approach will bring a clan four times
(x4) more Fame Points compared to the basic amount.
The numbers of FPs rewarded for capturing approaches with and
without the bonus for previously capturing the Citadel can be seen

Yes, Fortresses will appear from 29th November. 3. Yes, the bonus
applies to each approach and your example of 4 approaches giving a
potential 3 with x4 bonus is correct. Finally, all FP
rewards and bonuses go the Clan that achieves them, not just to the
individual players.
